Points are accumulated, grauously, by staying in the group's approximately 8,000 hotels around the world. What are Milestone Bonuses
Honors program milestones allow the user to receive bonus points upon reaching certain targets.

It is also possible to convert points from airline programs to the Honors loyalty program, but it is a practice that I only recommend as a last resort if you have points/miles expiring and rather than losing them it makes sense to transfer them to Hilton where the points have eternal or nearly eternal life.
